To report an emergency, DO NOT use this form. Call the University Police Department at (408) 924-2222, or dial 911.
By reporting discrimination incidents, you are helping to maintain ±«ÓãÖ±²¥ as an inclusive and safe campus environment. Once received, the Bias Incident Resource Team will review the information and may contact you for additional information. A process for review and response to submitted forms can be found here.
